Responsibilities of the Student for QLA Preparation
The QLA is to be completed by the student and is the sole responsibility of the student. Explicitly, the student preparation of the QLA will include the assembly of the pertinent references, the reading and interpretation of the papers, the organization, the formulation of a concluding engineering problem specification or hypothesis, and written presentation of the document. The student may seek outside assistance for editorial purposes only, e.g. the Purdue writing lab or review by peers. The general concept laid out in the report should not be an incremental improvement over a past MS thesis, dissertations of other lab members or grant proposals written by the advisor.
To the best of my abilities, the information presented on this form is accurate. In addition, I have read, understood, and will follow the guidelines as outlined in the statement of student responsibilities while completing the QLA portion of the qualifying student portfolio.

Responsibilities of the Major Professor (Advisor) for QLA Preparation
The submitted QLA document is written, organized, and prepared in its entirety by the student; the QLA is part of the Ph.D. qualifying process in BME and is not a written document for publication. Aside from the selection of the topic, the research advisor (major professor) plays a minimal role in this process. The advisor may address general questions from the student concerning the scope and content of the document and encourage the student to avoid certain topics or aspects. The advisor should not help organize the content of the document or help construct the concluding engineering problem specification or hypothesis formulation. However, the advisor may provide examples from literature or refer the student to existing examples of previously submitted and evaluated QLA documents.
