Files to restore:
List the names of files you would like restored. It is important that you specify which drive (e.g.: 'N:' or 'C:') contained the deleted files.
Example:
N:\Filename or N:\Folder\Filename or D:\Folder ( so all files in that folder )
If you have mapped a network drive, X:, you need to explain what X: is. For example, X:\Filename where X mapped to a share called "ecnshare"

List the names of files you would like restored. If you specify a directory, all files in that directory will be restored.
Example usage: /home/shay/a/yourlogin/Filename or ~/Directory
Machine name:
Note:
Your site specialist will contact you to help collect more information. Please provide any additional details below.
When were the files lost or ruined?
Because of the way backups are done, we need to know some details about the date and time the files were lost or ruined. For example, "I haven't changed the file in years, and lost it today [ today's date ] ." or "I wrote over the file on Friday, and didn't realize it was ruined until I tried to use the file today." If files were lost or ruined on multiple days, enter any known dates.
When did the last good version of the files exist?
It is helpful to know when you think the files were last in a good state. For example, "Files were OK yesterday [ yesterday's date ]", or "Files were OK for sure on [ date ]." You can also let us know how long the files have existed for weeks, months or years. If you don't know, just put, "I don't know".
