[Che-student-staff-list] Start of self-audits of ChE labs and offices

Hello Everyone,



Our School is scheduled to have the Integrated Safety Plan (ISP) review during the month of March 2023. Part of this process is the completion of the safety self-audit of ALL ChE laboratories AND offices.
The ISP Self-Audit checklist (attached) can be found on the ChE School's website (https://engineering.purdue.edu/ChE/aboutus/safety), under the "Laboratory Safety Requirements" heading (Integrated Safety Plan Self-Audit Checklist<https://www.purdue.edu/ehps/rem/documents/forms/ispcheck.pdf>) , or on REM's website: https://www.purdue.edu/ehps/rem/isp/index.html . Please only use these forms, as they are the most up-to-date forms that are now used at University level. One form needs to be completed for EACH lab/storage area/machine shop. However, multiple offices can be included on the same form.

  1.  The first eight sections have universal questions that apply to every area. For self-audits of office space, only section 1-8 have to be completed. I attach the Hazard Assessment Certification for FRNY offices for your information; please answer "yes" to questions 8.1 - 8.4 for offices.
  2.  Sections 9 to 29 have questions related to specialized operations; research laboratories and shops should address all sections (section 1 through 29).
  3.  All self-audit checklists must be signed by PI/supervisor, not necessarily by the person completing the self-audit.
  4.  Electronic signatures are accepted on ISP self-audit forms.



In addition to completing the self-audit for your labs/shop, please review and update the door posting, hazard assessment (should be placed inside the lab, near the entrance), chemical inventory (Form HCP4), and lab specific Chemical Hygiene Plan (at a minimum, the first page needs to be signed by the PI of the lab and dated within the last 6 months). They should be all accurate and dated within the last six months. Later this month the ChE Safety Committee will perform safety inspections of all laboratories and offices and will send the inspection reports to the faculty responsible for those areas. Deficiencies identified in your audits and during the safety inspections will need to be addressed before the recertification meeting with REM. The ChE Safety Committee (list of members posted on https://engineering.purdue.edu/ChE/aboutus/safety) is available to help you.
