[Me-indy-faculty] Lab/office access control: check in/out procedure

Dear colleagues,

As we are preparing for the upcoming lab move, I am sending you the instructions for get keys or card access to your labs/offices.

The access request must be sent by the PI so please do NOT have students or postdocs to submit the request. This means the PI is responsible for initial screening of access control.

The following procedure will be followed from now on. All access requests will be reviewed/approved by Departmental Facility Manager (not by the school).

The PI may request lab access by sending the following check-in information to Building Deputy (Tiffany Lewallen, tlewalle at purdue.edu<mailto:tlewalle at purdue.edu>) and cc Departmental Facility Manager (David Schippnick, dschippn at purdue.edu<mailto:dschippn at purdue.edu>).

  1.  Full name of the person who needs access
  2.  Email
  3.  Location of new lab
  4.  Purdue-Crimson Card information (#s located on the back of the card under the black strip)
  5.  Justification of request (why does this person need access)
  6.  Required Training? If needed
For check-out,  please send Tiffany and David name of each key holder and include serial # engraved on the key that needs returned. Tiffany will notify you how to return the keys.
Once the school notified PIs the office assignment for graduate students, the PI needs to request the access to the offices for his/her students.
