Center for Health Excellence, Quality and Innovation (CHEQI) - we are supporting the expansion of clinical pharmacy initiatives aimed at incorporating a wider range of social determinants of health into clinical pharmacy practice to better serve medically underserved populations. Lab times: Tuesday 4:30 - 6:20pm

We propose responding to the unique needs of patients with health disparities by developing a comprehensive resource tool that provides clinical pharmacists with evidence-based data and tools to impact health disparities. Through this initiative, we hope to positively impact the overall health outcomes of patients experiencing health disparities. 

  1. Develop a comprehensive, dynamic and accessible digital resource tool that informs clinical pharmacists on how to identify and address social determinants of health through a tailored approach.  
  1. Promote integration of the digital resource tool across national clinical pharmacist practice sites through strategic dissemination.  
  1. Establish mechanisms for continual updates and enhancements of the tool driven by user feedback and integration of emerging resources.
