How can I put missing “Table     Page” at top of page?

Put

\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
immediately before your \end{document} command. Then run LaTeX twice. If that doesn't work try putting more than five
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
\addtocontents{lot}{\mbox{}}
lines there (but not so many you get an extra blank page in your list of tables) and see if that works.
