The following list is an example of a teacher referral form used to evaluate kindergarten and first grade students for the elementary challenge program.

Teachers are asked to evaluate their child by answering seldom, sometimes, frequently, or almost always to the following questions.

• Expresses a wide range of interests and curiosity about almost everything that goes on around him/her.
• Asks many intelligent questions about topics in which young children do not ordinarily have an interest.
• Has keen observation and remembers information about things that he or she has seen.
• Has the ability to concentrate for a longer period of time than other children his/her age.
• Can tell time to the quarter of an hour using an analog clock.
• Can read and comprehend at the first or second grade level.
• Understands the meaning of numbers beyond 20.
• Has accurate use of large, advanced vocabulary.
• Has knowledge base of many facts and topics.
• Expresses him/herself well in entire sentences.
• Can tell or reproduce stories and events with great detail.
• Has creative, innovative ideas.
• Has an exceptional memory.
• Is motivated to learn and has task commitment.
• Learns quickly and easily.
• Has an advanced sense of humor.
