The questionnaire asks a variety of background information (such as name, address, etc.). It also asks the following questions:

1. What are your favorite subjects in school?

2. What is your least favorite subject in school?

3. How many books have you read in the past month? in the past year?

4. What are your favorite books?

5. Who are your favorite authors?

6. Name hobbies and/or activities that you enjoy doing during your leisure time.

7. Do you most enjoy working on projects by yourself or with other students? Explain your answer.

8. List/describe talents or skills you have.
