-          No open items. No changes to report.

4. Review of Open Near Miss Items

-       No Open Near Miss Items

5. New Open Near Miss Items

-       Several items relating to the WANG Building Emergency Plan were discussed:

o   Location of the fire extinguisher in the 3rd floor kitchen area

o   Glassed offices and rooms make shelter during an active shooting incident challenging.  Need better communication of safe areas.

o   The sign on the emergency evacuation exit is unclear.  Suggest wording of “EMERGENCY EVACUATION ROUTE – DOOR LOCKS BEHIND YOU.”

-       Students are accessing the barricaded areas under the stairs in the basement.  In one incident, a student entered the area for prayer.  The need for better signage and blockage, as well as the need for an alternative place for prayer were discussed.

-       The window cleaners at WANG were observed on the platform without fall protection using long poles to clean the windows.

-       Concern was raised about the new “hoverboards” students are beginning to use.  They will not be allowed to be charged in the classrooms due to fire risk, and they cannot be used inside the building.  Other student safety issues include running in the hallway, sliding down the bannisters, door opening in to the hallway, and proper storage of skateboards in the classroom.  A general safety slide for ENGR131 and ENGR132 was proposed.

Next Meeting: Friday, December 4, 2015 at 10:00 AM in ARMS B098
