I wanted to add additional information on how to confirm your flu shot to Purdue.

Please use the Healthy Boiler portal to do this, even if you received your flu shot from your own health care provider or local pharmacy. Whichever method you decide is convenient for you, they should provide documentation that you received your flu shot, be sure to keep that document as it will need to be uploaded to the link below. If you have never used the Healthy Boiler portal before, you will need to register a new account.

Dear All,

Per the Protect Purdue website, Purdue has previously announced that it has added the annual influenza vaccination<https://protect.purdue.edu/updates/flu-shot-details/> to the immunization requirements for enrolled students, faculty and staff (this includes postdocs and visiting scholars) as a condition to being on campus. This is a mandatory requirement in order to be eligible to be on campus as part of the pandemic response plan and Protect Purdue Initiative. Purdue is requesting that all students, faculty, and staff (this includes postdocs and visiting scholars) get their annual influenza vaccination regardless of whether on campus, working remotely, or attending classes remotely at any point in time. This is in accordance with what the CDC recommends that all people, age 6 months and older, get the annual influenza vaccination.

It is very important to understand that the requirement for the flu vaccine begins when flu season starts and Purdue has announced a few campus events to facilitate vaccinations, the deadline to get vaccination is currently planned to be by the end of October. Those who do not wish to come on campus for their vaccination may follow normal procedures to obtain a vaccination via their medical provider or pharmacy. As is usual, Purdue will allow exemptions for medical and religious purposes for the influenza vaccine but those employees will need to complete the Purdue University influenza declination form<https://www.purdue.edu/hr/COVID-19/flu-info/Influenza-Declination-Form.docx>. Faculty and staff must upload the declination form into the Healthy Boiler Portal<http://www.healthyboiler.com/>. Students will upload their declination form to the Student Health Center portal<https://www.purdue.edu/apps/account/cas/login?service=https%3a%2f%2fmyhealth.push.purdue.edu>.
