The scheduling office is now accepting evening exam requests for Fall 2025. Please fill out the evening exam request form via the link below and return it to me via email.

Important items to note:

**Please DO NOT SIGN the bottom of the form, the signature line is intended for the department head.

**Per the evening exam policy<https://catalog.purdue.edu/content.php?catoid=13&navoid=15965#scheduling-of-examinations> evening exams are not scheduled on Fridays.
